Two places you can report
ONC — Information Blocking
Use this when your provider's system didn't share your data with your app. The data should flow automatically — if it didn't, the provider may be blocking it.
OCR — HIPAA Right of Access
Use this when you asked for your records and didn't get them within 30 days, were charged too much, or your provider refused. This is your legal right under HIPAA.
